Pare che sia molto antica forse prima del 1000 ma allargata nel XVIII sec. colpisce il soffitto a cassettoni del 1600 molti i dipinti importanti e opere interessanti alcune riutilizzate dal passato. Flaviano era un patriarca di Costantinopoli del V sec. che vcondivide con San Vito il "Patronato di Recanati. Very interesting although it seems little appreciated, I liked it very much. Tanto per cambiare i restauri tardano. Peccato. Per arriva qui si vede questa parte di Recanati che sempre molto interessante. Consigliata la visita di questa Cittadina. We arrived which was already closed. Il presbiterio comprende l’altare maggiore "papale" e il coro ligneo. Lungo la navata di destra si succedono, dal presbiterio, gli altari di San Carlo Borromeo, San Filippo Neri, Madonna e santi, San Liborio. Lungo la navata di sinistra, a partire dal fondo della chiesa, si trova il battistero; di seguito sono le cappelle della Madonna Addolorata e del Santissimo Sacramento fornito di coro ligneo. Al lato sinistro del presbiterio si trova il sacello con le tre urne dei vescovi Angelo di Bevagna e Nicolò Dalle Aste, e del pontefice Gregorio XII. Contigua è la cappella delle Reliquie, tra cui da segnalare sono quelle della Santa Croce e il reliquiario con osso del braccio di San Flaviano, donati da papa Gregorio XII. LA CRIPTA conserva i resti della prima cattedrale: tracce di affreschi della Vergine Addolorata ai piedi della Croce, e di San Sebastiano; capitelli in pietra posti alla base delle colonne in cotto. Nelle due urne sepolcrali sono sepolti il cardinale Tommaso Antici deceduto nel 1812, e il vescovo Luigi Cossio scomparso il 3 gennaio 1957. San Flaviano is Recanati's cathedral and co-cathedral.
