Affascinante è non solo l'architettura barocca e neoclassica della chiesa ma anche la leggenda ad essa legata. Il 14 luglio 1691 durante una tempesta estiva un fulmine si scagliò accanto alla chiesa dove c'è la Porta di Mare proprio tra dei passanti. Nessuno rimase ferito. Il miracolo si attribuì all'icona della Vergine Maria già lì dal 1500 circa. Totally circular sanctuary. A real pantheon of Marsala devotion. Fascinating is not only the baroque and neoclassical architecture of the church but also the legend linked to it. On July 14, 1691, during a summer storm, lightning struck next to the church where there is the Porta di Mare right between passers-by. Nobody was hurt. The miracle was attributed to the icon of the Virgin Mary already there from around 1500.

Secondo la leggenda, durante una violenta tempesta, un giovane cavaliere si rifugiò sotto la Porta di Mare (oggi Porta Garibaldi), come tanti altri concittadini. Appena il ragazzo scese da cavallo, un terribile fulmine si abbatté proprio sul cavallo, uccidendolo, senza che nessuno dei presenti subisse il minimo danno. Ritenendo l'accaduto "miracolo compiuto dalla SS. Vergine" la cui statua era allocata in una nicchia nella porta, tutto il popolo, riconoscente, volle ringraziarla edificando la chiesa della Madonna del tuono per custodirvi la statua della Madonna stessa. La chiesa in origine aveva l’ingresso proprio sotto l’arco di Porta di Mare e una pianta allungata e irregolare. Nel 1750 la Congregazione dei servi di Maria SS. Addolorata ottenne dal Consiglio della città il permesso di trasferirsi nella Chiesa della Madonna del tuono o del fulmine, come veniva indifferentemente chiamata. Nel 1790 iniziarono poi i lavori di ricostruzione della chiesa, che da allora prese il nome dalla Congregazione, l’ingresso venne spostato nell’attigua piazzetta e la pianta divenne circolare. Dal 15 settembre 1997 la chiesa è stata elevata a Santuario Mariano Diocesiano. Oggi è luogo giubilare per acquistare l’indulgenza plenaria. Il venerdì santo la statua dell’Addolorata, insieme a quella del Cristo morto, viene portata in processione per le strade della città: uno degli appuntamenti più sentiti della settimana santa marsalese.
