★ ★ ★ ★ ★ L'edificio compare in alcuni documenti con la variante dell'intitolazione a Santa Maria Assunta in Cielo o all'Annunziata.
Una bolla del 16 marzo 1413 concede a frate Emanuele d'Incisa di fondare un convento nel suo paese d'origine, e probabilmente in tale occasione fu costruita anche la chiesa.
L'edificio fu ampliato in anni successivi (entro il 1487) e consacrato dal vescovo di Betlemme Cristoforo, come ricordava un tempo un'iscrizione posta all'interno della chiesa.
Accanto alla chiesa sorgono il convento, il chiostro e il cimitero.
Tra il XVII e il XVIII secolo il santuario fu ingrandito e arricchito di decorazioni in stile barocco.
All'interno sono interessanti:
- le pregevoli pitture murali risalenti a diverse campagne decorative quattro e cinquecentesche.
- il Polittico tardo gotico, opera di scuola ligure.
- il Crocifisso ligneo, opera del XV secolo.
- la statua lignea della Madonna col Bambino sormontata da una corona. La statua è stata restaurata nel 2003.
 The building appears in some documents with the variant of the dedication to Santa Maria Assunta in Cielo or all'Annunziata.
A bull of March 16, 1413 allows friar Emanuele d'Incisa to found a convent in his country of origin, and probably the church was also built on that occasion.
The building was enlarged in subsequent years (by 1487) and consecrated by the bishop of Bethlehem, Cristoforo, as was once remembered by an inscription placed inside the church.
Next to the church there are the convent, the cloister and the cemetery.
Between the seventeenth and eighteenth centuries the sanctuary was enlarged and enriched with Baroque decorations.
Inside they are interesting:
- the valuable mural paintings dating back to various fourteenth and sixteenth-century decorative campaigns.
- the Late Gothic Polyptych, a work by the Ligurian school.
- the wooden Crucifix, work of the fifteenth century.
- the wooden statue of the Madonna and Child surmounted by a crown. The statue was restored in 2003.
