La Madonna di Piedigrotta, di cui è presente la statua lignea, ha una storia affascinante e raccontata egregiamente da Benedetto Casillo durante le Feste di Piedigrotta. In molti chiedevano alla Madonna la sua protezione o il suo conforto, numerosi sono infatti gli ex-voto a lei dedicati anche nei periodi più bui. Nel 2015 durante le feste è stato possibile apprezzare una mostra storico fotografica sul tema. Ogni anno la comunità parrocchiale organizzare un pranzo per gli amici di strada in occasione dell'Epifania. E numerose sono le iniziative intraprese dalla comunità stessa. La Chiesa negli ultimi anni sta subendo un'opera consistente di restauro degli altari e della facciata grazie all'impegno dei Canonici Regolari Lateranensi e dei fedeli. Il nuovo impianto di illuminazione consente finalmente di godere di una chiesa luminosa e lucente. All'interno si segnala monumento funebre della famiglia Filangieri, con una grande statua dedicata a Gaetano. A basilica rich in history, culture and popular religiosity. Il nome significa ai piedi della grotta perché è da qui che parte il tunnel che porta nella zona di Fuorogrotta. L'attuale facciata, dove si trovava un affresco di Giacinto Gigante del 1853, poi perduto, fu realizzata da Errico Alvino mescolando linee rinascimentali ad altre gotiche. Il portale in noce fu scolpito da Bernardo Manco nel 1853 e reca i quattro evangelisti e i SS. Apostoli Pietro e Paolo.
