Edificata nel 1735 al posto della chiesa di Tremonti perché si tardava a ricostruirla dopo il terremoto. Rimasta chiusa per tanto tempo, è stata restaurata e riaperta al culto nel 1986. All'esterno si trova un antico affresco della Madonna del '700 appartenuto all'edicola votiva là esistente ancora prima della chiesa stessa, nel posto ora dove sorge a chiesa, per anni meta di tanti pellegrinaggi per le numerose grazie li ricevute dai fedeli. Attualmente la chiesa ospita l'adorazione eucaristica perpetua.
