The Basilica of San Gennaro Fuori le Mura is located in Vico San Gennaro dei Poveri, it owes its name to the place of construction, at the foot of the Capodimonte hill, chosen outside the city walls to keep it away from the pagan world, it is the oldest early Christian church dedicated to San Gennaro. Its construction dates back to the 5th century, in the area of ​​the Catacombs of San Gennaro, probably as a result of the union of two pre-existing cemetery sites from different eras: one from the 2nd century in which the body of Sant'Agrippino, the first patron saint of Naples, was preserved. , and another from the 4th century which housed the remains of San Gennaro until their first translation in the 9th century. The building represents a testimony of early Christian architecture, even if it has undergone some changes in the centuries following its construction, still visible in some features of the structure, especially between the ninth and fifteenth centuries. In 1892 the vault was replaced with a trussed ceiling, while in a restoration of the early twentieth century an attempt was made to recover the original structure by eliminating some changes made previously. The interior, with a structure with three naves, recalls the late Gothic style and has an early Christian semicircular apse, supported by two Corinthian columns. The church is preceded by an entrance hall, decorated with sixteenth-century frescoes, representing episodes from the life of San Gennaro, attributed to the painter Andrea Sabatini, active in Naples between the second and third decade of the sixteenth century.

The La Paranza cooperative, made up of young people from the Sanità district, reopened this church in 2008, which for years had become the landfill of the nearby hospital, and returned it to the community. Although it is consecrated, it is used as an exhibition venue for exhibitions or events. It is located just outside the exit of the Catacombs, of which it was originally the entrance. Built in an early Christian form in the 5th century, it was abandoned after the theft of the remains of San Gennaro. In the 15th century it was renovated and adapted to the Baroque style. In place of the nearby monastery, a hospital for plague victims was built (San Gennaro dei Poveri), later a hospice for the poor. A restoration of the twentieth century has brought it back to its original forms. The atrium frescoes are beautiful, probably by a follower of Raphael.
