The building is located to the south-east of the mother church, in a small space of the homonymous street in the town, together with some eighteenth-century portals and during the Middle Ages it was called di San Nicolò. It is a church already present in the fourteenth century, and used as a sinecure chapel, as evidenced by some documents from the years 1390 - 1391. According to tradition it was dedicated to St. Nicholas of Tolentino and is perhaps even older than the church of Itria. It was administered by the priest Antonio De Bona, who also managed the parish of San Giovanni de Puteo or Santa Caterina and the sinecure chapel of San Lorenzo, which was located inside the church of San Nicolò di Naro. The chapel of San Nicolò di Favara and that of San Lorenzo di Naro, gave him about 6 gold florins a year. The structure has a simple baroque façade on the outside with a pointed entrance portal, while inside it is divided into a single rectangular nave. In 1584, a hospital for the sick, active until the 18th century, was annexed to the church.
