Il toponimo è ricordato come "Submontio" (Semonzo) e ne indica la collocazione alle pendici del Monte Grappa. La Chiesa attuale rappresenta il risultato dell'ampliamento e delle modifiche strutturali interne, delle rifiniture e delle opere pittoriche, lavori che iniziarono nel 1750 e si conclusero nel 1775. Nella nicchia dell’altare si trova una statua della Madonna con Gesù Bambino in braccio che porta lo scettro in mano. La statua venne poi ricollocata in chiave moderna nel 1992 con aggiunti i simboli dello sport del parapendio invocandola come Madonna del Buon Volo.
