Di quel primitivo edificio romanico poco è rimasto anche perché fu devastato da Federico II nel 1248. Duecento anni dopo il duomo fu però ricostruito in stile francescano e poi ancora trasformato alla fine del 1700 in stile barocco ed all'inizio del 1800 in stile neoclassico. Al '700 risalgono anche le sei cappelle ricavate nelle pareti laterali che ospitano opere pittoriche soprattutto di Giulio Cesare Angeli eseguite a cavallo del XVI e XVII secolo. La chiesa può fregiarsi anche del titolo di "cattedrale" in quanto fu sede vescovile e conserva le spoglie di San Rinaldo, patrono di Nocera.
