Secondo alcuni storici, già prima della costruzione del Castello Ezzeliniano, preesisteva un sacello votivo dedicato alla Vergine; ma ben più fondata è l'ipotesi che sul colle dopo la strage ezzeliniana del 24 agosto 1260, venisse eretto in suffragio dello sterminio perpetuato, una piccola chiesa dedicata alla Madonna. La direzione sarebbe stata assunta da una confraternita di Battisti, con lo scopo di espiare i peccati. In seguito ci fu un sacerdote proprio e dal 1608 il titolo di Rettore venne assunto dal parroco di S. Zenone. "Santa Maria del Castello" - così durante il tredicesimo secolo si chiamava la chiesa - subì due gravi danni durante il terremoto del 1695. I lavori di restauro non si fecero attendere e la chiesa fu rimessa a nuovo, ma un secondo più violento sisma, nel 1836 la divelse quasi completamente. Il desiderio intenso di Mons. Antonio Bianchetto (parroco di S. Zenone, 1873-1894) e della popolazione di ricostruire il santuario divenne realtà nel 1891 quando su progetto del prof.
