★ ★ ★ ★ ★ Piccola chiesa campestre, sorge isolata su un poggio posto sotto l’abitato di Sant’Urbano.
L’impianto è quello tipico dell’edificio votivo cinquecentesco, con due finestre laterali e il portale in cotto coperto da una trasanna, sopra la quale è posto un oculo a dar luce all’interno.
Il campanile a vela, a un solo fornice, è disposto centralmente, in linea con la facciata.
La copertura è a capanna.
Interno
L’interno è a una sola navata, con la parete di fondo interamente affrescata.
In alto v’è un’Annunciazione, con a sinistra l’Angelo annunziante, a destra la Madonna Annunziata e al centro Dio Padre benedicente.
Al registro inferiore a sinistra Sant’Onofrio, sull’arco a sinistra un Santo Vescovo, al centro una Crocifissione, col Cristo tra la Madonna e San Giovanni, sull’arco a destra ancora Sant’Onofrio,poi una figura quasi totalmente guasta e non riconoscibile.
La chiesa versa in grave situazione di dissesto e sarebbe già crollata senza la puntellatura.
A quanto risulta la Curia ne sta riacquisendo la proprietà e procederà senza dubbio al restauro.

 Small country church, stands isolated on a hillock located under the town of Sant’Urbano.
The layout is typical of the sixteenth-century votive building, with two side windows and the terracotta portal covered by a sweat, above which there is an oculus to give light inside.
The bell gable, with a single archway, is centrally arranged, in line with the facade.
The coverage is gable.
Indoor
The interior has a single nave, with the back wall entirely frescoed.
At the top there is an Annunciation, with the announcing Angel on the left, the Annunciation on the right and God the Father blessing in the center.
In the lower register on the left Sant'Onofrio, on the arch on the left a Holy Bishop, in the center a Crucifixion, with Christ between the Madonna and San Giovanni, on the arch on the right still Sant'Onofrio, then a figure almost totally damaged and not recognizable.
The church is in serious distress and would have already collapsed without the shoring.
Apparently the Curia is reacquiring the property and will no doubt proceed with the restoration.
